Obama’s Rationing Man

President Obama’s nominee to run Medicare and Medicaid has declared his love for Britain’s health care system. Literally.

While much of Washington is focused on President Obama’s Supreme Court pick, Republicans are gearing up for a confirmation battle over another Obama nominee who promises to put health care back in the spotlight.

At issue is Obama’s choice to head the Centers for Medicare and Medicaid Services, Donald Berwick, a Harvard professor with a self-professed love affair with Britain’s socialized health care system. In his writings and speeches, Berwick has defended government rationing and advocated centralized budget caps on health care spending.

“Cynics beware, I am romantic about the (British) National Health Service; I love it,” Berwick said in a July 2008 speech at England’s Wembley stadium. “All I need to do to rediscover the romance is to look at health care in my own country.”

While Berwick would not have the authority to impose a British health care system on the United States in one fell swoop, as head of CMS, he would be running both Medicare and Medicaid. Given that the two programs alone account for more than one out of every three dollars spent on health care in America (all government programs combined account for 47 percent), private players tend to follow CMS’s lead. Berwick himself has made this point.

“(G)overnment is an extraordinarily important player in the American health care scene, and it has inescapable duties with respect to improvement of care, or we’re not going to get improved care,” he said in a January 2005  interview with Health Affairs. “Government remains a major purchaser.… So as CMS goes and as Medicaid goes, so goes the system.”

There are two basic visions for how to contain the growth of health care spending. The free market approach would give individuals control over their health care dollars, with the idea that it would encourage more shopping that will drive down costs and increase quality as has happened in every other aspect of the consumer-based economy. But the other approach, employed by nations such as Britain, is to have the government ration care to meet a global budget.

President Obama rejected the market-based approach, and sought to drastically expand insurance coverage while reducing health care costs. But according to a report by CMS’s chief actuary, the new law will actually increase health care costs. That leaves rationing of care based on a bureaucratic notion of the common good as the remaining option for containing skyrocketing spending, and it’s an outcome that Berwick himself once predicted would be necessary to achieve universal coverage.

“(T)he Holy Grail of universal coverage in the United States may remain out of reach unless, through rational collective action overriding some individual self-interest, we can reduce per capita costs,” Berwick wrote in an article for Health Affairs he co-authored in 2008.

He went on to write that, “The hallmarks of proper financial management in a system… are government policies, purchasing contracts, or market mechanisms that lead to a cap on total spending, with strictly limited year-on-year growth targets.”

On a number of occasions, Berwick has praised Britain’s National Institute for Clinical Excellence (NICE), a body of experts that advises the government-run health care system on how to allocate medical spending based on cost-benefit analysis. Among other decisions, they have ruled against the use of cancer-treating drugs and put a dollar value on the final six months of human life.

“NICE is extremely effective and a conscientious, valuable, and — importantly — knowledge-building system,” Berwick said in an interview last June in Biotechnology Healthcare. “The fact that it’s a bogeyman in this country is a political fact, not a technical one.”

The national health care law that President Obama signed in March will greatly expand the role of CMS by adding an estimated 15 million beneficiaries to Medicaid. In addition, the law contains a number of initiatives, to be spearheaded by the Secretary of Health and Human Services in conjunction with the head of CMS, to provide incentive-based pay to doctors and hospitals based on performance. This builds on the comparative effectiveness research provision of last year’s economic stimulus package. While none of these measures will have the same sway as NICE does in Britain, taken together, they will move America in a NICE-like direction, especially with Berwick at the helm.

In 2003, Berwick signed on to an open letter in Health Affairs, called “Paying for Performance: Medicare Should Lead.” (Among his co-signers was Nancy-Ann DeParle, the current White House health care czar.) “Our recommendation-to the executive branch; to Congress; to employers and health plans; and to hospitals, physicians, nurses, and other health professionals — is that payment for performance should become a top national priority and that Medicare payments should lead in this effort, with an immediate priority for hospital care,” the letter read. It went on to say that the CMS administrator’s successors must continue to show “aggressiveness and commitment” to the cause, noting that, “A major initiative by Medicare to pay for performance can be expected to stimulate similar efforts by private payers…”

The idea of paying doctors and hospitals for delivering better quality health care and of offering guidance on best practices seems benign enough. As the letter put it, “Quality is not an issue for partisanship.” The problem arises when government bureaucrats or expert panels are in the position of judging quality, performance, and best practices which get applied across a broad and diverse population.

As cardiologist Sandeep Jauhar argued in a September 2008 New York Times op-ed, pay for performance initiatives can cause unintended consequences such as doctors overprescribing certain medications that are deemed effective and carry bonuses. He also recounted how an initiative in the early 1990s to give report cards to doctors performing coronary bypass surgery prompted doctors to cherry pick patients to avoid the most severely ill cases that could jeopardize their grades.

When I was younger, I used to think Harvard and those other Ivy League schools were something to be admired. As it turns out I was wrong because all it turns out is freaks and Marxists who know they will end up going to hell because they believe in no one besides Big Brother.

Berwick is another sicko, in the same image of Barack "Insane" Obama, who thinks life is over-valued, unless of course you are one of them.

In November, and this should be the catch phrase daily to voters: Do you want these people to control your lives ? Put their phase along with their names. The way to gin up motivation is to get voters angry. The Democrat Party is giving ample opportunity to do this.

There is nothing that frosts me more is having a unelected or an appointed bureaucrat who is wildly in love with, "Well, this is the way the Europeans do it."
The self-proclaimed Lord Mayor Bloomberg is currently in London admiring that city's gazillion cameras that monitor ever single citizen moves.
His Lordship will predictably come back and demand that the tax payers shell out millions upon millions of dollars to put cameras in every conceivable orifice of New Yorkers.
Donald Berwick and all the other Haavad proffesors need to be reminded by the American people that this Republic is not to be run like a college frat house or their socialist experiments.
These United States are not wanting to join the European Union, not now, not ever, and hell can freeze over before Americans would even entrain the European empowerment.
This is why millions of our great grandfathers and grandmothers left the damn place to begin with.

How do you cost/benefit an elderly person's health needs when the elderly do not live in vacuums? I am thinking of the broken window aspect of economics applies to medical economics also. It isn't what you see (the cost of an elderly person's direct costs) but what you don't... the resources a family would be required to bear to compensate for denied care. For example: A seventy five year old requires a hip or knee that would keep them active, keep them mobile, but is denied (individual estimation of quality was derided by Obama's take a pill perscription using his own grandmother's hip replacement as an example). The loss of the elderly's mobility means either the family has to use their time being the parent or grandparent's legs, or nursing home care which is much more expensive. An active 75 year old can also provide needed support and help to busy families, I know a lot of them that are blessings to their families.

Isn't government rationing of medical care just another version of eugenics? Utopians are always trying to weed the human garden, and economics is just an excuse to eliminate those they deem not acceptable.

All health care is rationed. Just like any other resource it is not infinite. Govermnent rationing of care is the logical conclusion of government payment for care.
Perhaps, the family should pay for the hip replacement for grandma themselves? It would eliminate the need to be approved by a government insurance board. Of course the eventual conclusion of our slow slide into socialistic medecine will be the inability to pay for one's own healthcare out of pocket. But that is what happens when the country elects a horde of Democrats.

I think you don't realise the inevitable results of a government takeover of medical care, and the rationing that will also be inevitable. It is fine to say that the family should pay for the operation, but the fact will be that, they won't be allowed to, even if they have the money. Private solutions will not be permitted, and indeed, will be illegal. You will get what "the goernment" says you can have, and nothing else.

Sure, market forces work fine when I'm buying a pair of pants, shoes, potato chips or even a car, and the car is debatable. These are all things that we all understand work for us or not, we like or we don't. When it comes to healthcare and legal matters, where we depend on the professionals to help us, market forces fall apart. Most, if not all of us, could not argue with our doctor about our need for an MRI or a new medication, or even an operation. Sure, we can get another opinion, but on what basis do we decide? We are simply ignorant of what is the best course of medicine for us. The need for an attorney is similar, in that we don't understand the nuances of the law in most cases, and need professional advice to help us through the maze of a lawsuit or real estate transaction.

Moreover, free market principles caused our latest financial breakdown, where the Government regulations or watchdogs were not sufficient enough, including Freddie and Fannie, to stave off the country nearly going over the cliff. But the market made tons of money, didn't it?

Market forces are powerful, but they are not kind, not respectful of stability, and when ultimately unfettered, are not good for the country.

It is the expert panels that decide the generally accepted practices for physicians regarding specific maladies. Someone puts some sense and standardization to our medical practices now. Bringing them all together under one central expertise makes sense and will save money in the long run providing better care for everyone.

But - if you want you can always buy more medical care, if you think you are special.

The patient makes his 'market decisons' based on information provided by a primary care-giver, and, if he deems necessary, second opinions obtained by himself, or a guardian if need be, on the open market. You need not "argue with [y]our doctor" about a need to for an MRI, you, and your doctor need options about where to obtain that MRI on a cost-effective basis. Mandates are never cost-effective.

Under-regualtion was not the cause of our recent "financial breakdown"; lenders writing bad notes because they had a guaranteed market for them, Fannie and Freddie was the root cause. That market was provided by our Federal governors, those you want hand even more power.

No one, not of any authority certainly, has proposed "unfettered" markets. Please provide something to substantiate that misdirection if I'm wrong.

So, you think placing care-giving policy-making even farther from the patient is a good idea. Thank you, Dr. Orwellion. I guess it's not a village, but a thousand-mile-away-bureaucrat that's needed to raise a child now.

It was not removal of regulations it was deliberate government policy to drive home ownership rates ever higher in the pursuit of social equity that led to the financial meltdown. The bad loans are never made in a functioning market because the risk was too high. Both implicit and explicit threats from regulators upon banks drove this market; artificially low interests rates fed the bubble. Fanny and Freddy and their implicit government backing led the way. The meltdown was very much the logical if unseen conclusion to government failed government regulations not the result of lack of regulations.

Sure there were bad loans, but that WAS not the main reason for the meltdown. We wouldn't have had nearly the problem, had individual loans not been sold by the originating lender, and they had kept some responsibility for the loan on THEIR books - by enabling the passing of risk of the loan to others, they had less incentive to only make good loans. Instead they packaged and sliced and diced the loans to create the shadowy derivatives market, that are UNREGULATED, opaque, and 69 Trillion dollars at risk, while Wall Street and the Big Banks were leveraged way beyond what was prudent. Smaller, community banks did not have the same problems most of the big banks and big investment firms did. Remember, Lehman Brothers, Bear-Stearns, Merrill Lynch are all gone, and Goldman-Sachs and Morgan Stanley are now bank holding companies.

Watch "House of Cards" by David Faber, on CNBC, sometime if you don't believe me. Or get his book. It is clear that unbridled greed caused the meltdown. The poor people that were stuck with the sub-prime loans were victims as much as anyone. They did take the loans, but they didn't cause the meltdown.

That is why we need solid financial reform and re-regulation of the system, including regulation of the shadow markets so everything is visible and on the up and up.

I know you want to blame the Government for all the ills in your life, but kids it's both. Lack of Government regulation AND unfettered free enterprise, which leads to GREED run amok. The 1920's showed us what over-leveraging can cause, and we did it to ourselves again. We repeated history, but we did it in new and fanciful ways with new & complicated market instruments, with Government's help, ignorance and dereliction of their duty. But, GREED on all sides caused it, and we need to put circuit breakers and walls in place to stop it from EVER happening again.

And, btw, by instituting Free Market Obamacare, the Healthcare industry will have brakes and stoppers placed on the unbridled growth of medical costs, again GREED in the name of service to the masses run amok. You should all read T.R. Reid's book "On Healthcare" ... you'll learn something.

About Social Darwinism... I believe you have the facts mixed up. Generally, conservative Christians like me don't buy into that sort of tripe. Social Darwinism is only supported by a mindset that rejects the establishment of civil society and seeks to change it through the Hegelian dialectic process. It is the socialists/statists in our society that are bringing about rapid societal change, but you seem to reject this.

Unions alone did not make "benefits" happen, because companies and unions developed the concept of benefits during WWII when pay was capped and said companies wanted to find ways to entice people into work. Unions generally solidify guarantees of said benefits because they force companies to enter into contracts which usually include yearly pay raises, health care insurance options and the like. The U.S. has been and currently subsidizes insurance through companies like this, and this is a flawed practice that must be stopped. The new HCR legislation doesn't change this, but forces companies and individuals to comply with a new mandate in the flawed system so we can all reap the benefits of increased premiums... Yay! I'm not in favor of single-payer either, though some individuals seem to suggest the conservative mindset would support it because it would be more amenable to crap we already have in place like Medicare. Government-run health care can only cut costs through rationing, thus the elderly and the very sick are those most abused by the government system.... Look at the new boss, same as the old boss.... The exchanges in the plan sound interesting in themselves, but they will be heavily regulated by the U.S. government and therefore perhaps unsuccessful. We shall see.

I don't know about the martial law statement. God-fearing, law abiding citizens generally don't have to fear for much inside the confines of the civil society. It is the society engineering Statists within and without our government that seek to abolish the civil society and bring chaos to the United States on the order of "transforming" our society. New Age Racism, abortions, euthanasia, these are all the relics of increasing societal agitation brought about by socialists/statists. The statists in our society play the populace against itself through factions divided by race, religion, etc. The second bill of rights proposed by FDR is a Statist dream, which can only be accomplished by a much more socialistic society. Not unlike communism the government must have control over the individual to make such dreams possible.

You're the one who seems to embrace this ideology. You're on board with Obama, and that makes me question your stated "Centrism." He is not pragmatic because he is a demagogic statist ideologue, and the statist only knows that the government must be the agent in the "perfection" of society.

Historically, the controls placed on the financial industry coming out of the Great Depression provided a stable financial system for us for almost 80 years. Of course, money men chafed at the rules and regulation. So what? It was only after Reagan was elected that we started dismantling the safety provisions given us by FDR. The dismantling continued until the final blow of recinding Glass-Steagle in the Clinton Admin, a move President Clinton now regrets. Prior to the 1990's the financial industry was approx. 10% of our economy; in 2007 it was near 50% of the economy. And, since the financial industry doesn't make anything, we have lost manufacturing and are losing the ability to make "things" that we and the world needs. Who needs another Credit Default Swap? It's a House of Cards that was built on the Greed and Selfishness of Wall Street and the Big Banks.

Yes, the Government had a hand in the breakdown in 2008, but the biggest change, and the most responsible action that the Government is to blame for in this crisis is the lack of regulation and the holding back of the Free Enterprise, Laissez Faire, un-bridled Greed of the last 15 years or so.

There is lots of blame to go around - homeowners signing up for loans they couldn't afford and individuals borrowing on credit to the hilt, for sure have a piece of this. But it was the Big Boys that know better - Wall Street, Big Banks, the Fed and the Government that failed us all.

We must re-regulate the whole damn thing to get back on an even keel. Too bad all the people that remember the financial crisis in the 1920's and 1930's are dead now - Apparently their collective wisdom went with them to the grave.

I'm a physician and I think you are wrong. Most medical issues are pretty simple and understandable by people with a high school education. With so much information available through the internet there is no excuse for ignorance. Most life decisions like career, house, finding a mate, schools for kids are harder but we don't surrender them to government. We spend hours on line obsessing over the details of our next consumer purchase but don't even question our doctors about our medical care? That's crazy. I think people are smarter than that. I would love to have patients who with sit with me and work out cost-effective answers to their problems instead of sitting like dumb rocks swallowing whatever we tell them. But they don't pay for anything and so have no incentive to ask questions and therefore we have no incentive to practice cost-effective medicine.

I trained in single payer (Ireland) and it is a bureaucracy that robs people of their human dignity. Now I work in the US. You have the most productive, innovative and best medical system in the world. Don't destroy it to correct "crises" invented by liberals. Figure out the cost issues, extend coverage to those need it and leave the rest alone.

It's not that people are dumb, although when a doctor starts spouting medical terminology, even the brightest can feel stupid. If you are a doctor, and I have no reason to doubt it, then tell me this. How many minutes do you spend with the average patient? 10? 15? It can't be more than that, can it? How is someone supposed to weigh the medical issues and see multiple physicians (which is extra cost in itself) to find the cheapest procedure? That just doesn't happen.

If you don't have any money and no insurance, what did people do back when that was the case? They did without, didn't they? And, they died much younger of preventable or treatable disease.

The trouble is, along with better access to health care and a longer life has come the bill to pay for that better care and healthier life.

Tell me, who is going to regulate the cost of medical care? Insurance companies controlled their costs by throwing people off insurance when they got sick, or not insuring them in the first place.

How should it be done? Do you want your medical care to have the same consequences as your financial position - in the hands of the free market? Please tell me y'all are smarter than that.

I have to admit to you that I agree the health insurance model is a horrible one because the customer generally doesn't review cost vs. benefit, and that market forces driving prices are imperfect at best. Personally, with almost no expertise when it comes to medical care, I must look to others for answers and advice. There are a lot of talking heads out there, but seemingly the more rational ones are telling us to think and worry about the future of our country and our medical care.

The costs of our current entitlements and other care are driving us into a VERY deep debt as a nation, the future outlays for which are going to cost us some pretty cool trillions. Obama et al told us that this new bill would lower the deficit, but the math of adding people to a broken system and claiming to lower costs doesn't match... Unless we start rationing care, fast.

What is your proposition?

I've been watching the old British miniseries "Dr. Finlay," courtesy of Netflix, and have been bemused to see the debate about their health service included in its dialog. Set at the end of WWII, the show centers on village physician Dr. Finlay, who is looking forward to the National Health Service.

His fellow physicians believe that the system will make them employees of the government, that care will be overseen by bureaucrats, and that patients and physicians will pay a huge price for it all, in more ways than one. Finlay sees NHS as the way to provide care to everyone, no matter their income or status. The series, based on books by physician author A. J. Cronin ("The Citadel"), seems to want us to take Finlay's side against his reactionary, progress-fearing colleagues.

First go read what the divisions were between the Colonies and what was the main stickler to the signing of the Constitution. The 2 main arguments were the small states being afraid of the power of the bigger states - ergo the Senate came about to provide 2 Senators for each state. The second argument was over Slavery in the United States. They eventually compromised and allowed the language of "3/5 persons" to be allowed, without specifically naming the word Slavery.

Obviously, Abraham Lincoln was a Republican, but that Republican party bears no resemblance to today's Republican Party. Party affiliations have shifted many times in our history.

The Republicans of today's South were the Democrats of pre-1965. If you all knew you're history you'd know that. Prior to 1965, the Democratic party had a lock on the South, and yes, were segregationist, but with the Johnson Administration passage of Voting Rights and Civil Rights acts of 1964 and 1965, the Democrat South jumped to the Republican Party. Nixon called it his "Southern" Strategy, and Johnson said at the time "There goes the South" ... knowing the the White South would switch parties, they did and here we are.

As far as Sodomy, if you aren't aware of all the hypocrisy on the Republican Conservative side, where they preach family values while at the same time cheating on the wives by screwing little boys and young men and lying about it, you aren't paying attention. Sure Democrats do it too, I'm sure, but they're not preaching how much holier than thou they are, like the Conservative sex fiends while shtuping their boyfriends.

Sedition is an interesting case, since there is no such thing in the United States.
